Campaign Contribution Disclosure Form (Online Version)
NOTICE: Electronic Filing of this Campaign Contribution Disclosure Report is not a substitute for filing a signed, notarized hard copy of the report by the required due date with the Elections Division.
Submission Instructions:
When you click the Submit Form button you will be given 3 options to submit the form.
Choose option 1 if you use a desktop email program like Microsoft Outlook. You will then click Send Data File. Your email program will open and allow you to send the email to Pamela Coman and Shirley Arnold of the Registration and Election staff.
Chose option 2 if you use an online email program like Yahoo or Gmail. You will then click Save Data File and follow the instructions in the pop up box.
Choose option 3 if you use something a program other than the previous mentioned. You will then click Save Data File and follow the instructions in the pop up box.

Every public officer who is an elected county official and every candidate for election as a county public official will file a Financial Disclosure Statement with the election superintendent of the county of election. |
