Our Mission Statement


Fulton County Department of Health and Wellness Vital Records Office is responsible for filing, preserving, protecting, and issuing certified copies of birth, death certificates for events that occur in Fulton County. This Web site provides information on obtaining copies of vital records, and other available services.
 

Please note the following: By state law, Fulton County Department of Health and Wellness Vital Records Office requires a written application and non-refundable fee for searching for any vital record. We do not provide free searches or verifications, such as confirming that a certificate is on file or verifying the date of an event. The Fulton County Department of Health and Wellness Vital Records office can issue birth certificates for other Georgia Counties in most cases even if the records are in an another county in the State of Georgia.

  • By state law, the Fulton County Department of Health and Wellness Vital Records Office cannot accept telephone orders for birth certificates or other vital records.
  • This site provides instructions for requesting copies of birth and death, certificates. The fee is established by state law and is for the search for a certificate; if a certificate is located, the first copy is issued at the cost of $15.00 and $5.00 for each additional certified copy, provided that they are requested at the same time.

Fees for copies of vital records will be collected at the time of the request. Fees for birth and death records are the same as those charged by the State Vital Records Office.
On-line applications can be placed through a company called Vital Chek. For expedited service, contact Vital Chek information line at the toll-free phone number, 1-800-255-2414, 24 hours a day, seven days a week. Documents can only be requested through the Vital Chek web site; however, there is an additional $8.50 charge for the use of this service. To further expedite the request, overnight services are offered.

Who Can Ask For a Birth Certificate?

  • Registrant-person listed on birth record
  • Either parent-name must be on certificate
  • Legal Guardian-must show proof
  • Grandparents-if paternal, father's name must be on certificate
  • Adult brothers or sisters-must be 18 years old or older
  • Adult child-must be 18 years or older
  • Husband/wife-must have marriage license
  • Legal representative-of any of the above with proof
  • If you do not meet one of the above criteria, you cannot receive a certified copy of a birth certificate.

How Do I Request Copies of Birth Certificates?


 
 
There are two ways you can obtain a copy of a birth certificate. You can either apply on-line through Vital Chek or in person for a copy of a birth certificate at:
 
Aldredge Health Center, 99 Jesse Hill Jr. Drive, Room 101, in downtown Atlanta. The office is open from 8:30 A.M. until 4:30 P.M. (E.S.T.), Monday through Friday, excluding legal holidays. Click here for the driving directions. Applications are available in our office and must be filled out in duplicate.

Identification ("ID") is required to pick up certified copies of vital records in person. No copies of ID or expired ID will be accepted. The acceptable forms of identification are listed below.

Requires ONE of the following:
  • Government-issued employee I.D. badge with photo
  • Driver's license, any state with photo
  • State issued I.D. card with photo
  • Social Security Card
  • WIC Voucher
  • Monthly Medicaid Card
  • Insurance card/policies
  • Georgia utility bills
  • Credit Card
  • Payroll Check Stub
  • W-2 Form
  • Completed Income Tax Return
  • Passport
  • Military ID
  • Automobile title/registration
  • Voter registration card
OR Requires TWO of the following:
  • U.S. passport
  • Checkbook/bankbook
  • Major credit card
  • Health insurance card
  • Recent dated, signed lease
  • Grady Hospital Card
  • Check cashing card
  • Library card
  • Certified ID issued at local store
  • Utility bill or traffic ticket
All forms of identification must show your current name and address. Expired cards or documents will not be accepted.

You have to send a photo copy of your present identification if the certificate copies are mailed to you. There is no additional charge for postage.

The cost for one copy of the birth certificate is $15.00. Additional copies of the same record, purchased at the same time as the first, are $5.00 each. This cost is for a certified copy of the birth or death certificate. Acceptable methods of payment for in-person requests are cash, personal check, or money order. Credit and debit cards are not accepted for in-person requests

What if I don't have all the information requested in the application?


For birth certificates you must provide all the information requested in the application. For a death certificate, you must provide the name of the subject at the time of death and the year of death.

What is a Certified Copy?


A certified copy of a birth certificate issued by the Fulton County Department of Health and Wellness Vital Records Office will have a raised seal, will show the signature of the State Registrar, and will be printed on security paper. A certified copy is required to obtain a state-issued driver’s license or identification, a passport, or insurance benefits, or to travel to some foreign countries.

What Information Will be on the Birth Certificate?


We only issue certified copies of a birth or death certificate on file.
The certified copy will show the name of the subject at birth, date of birth, place of birth, parents’ names, file date, and certificate number.

The certified photocopy will be a copy on security paper of the original certificate. It may contain additional information such as time of birth. Many birth certificates were handwritten and may be difficult to read. If changes have been made to the certificate, the original information may be crossed out and new information written in. You may need a certified photocopy of a birth certificate if you require an apostille or are planning to adopt a child from a foreign country. If you require a certified photocopy of the birth certificate, please specify that in your request. If you have questions concerning this subject, please contact our office.

How Do I Obtain a Birth Certificate if the Birth Occured in Another State or Another Country?


Fulton County Department of Health and Wellness Vital Records Office has certificates only for births that occurred in Fulton County. You can find information on obtaining certificates from other states or U.S. territories on the Internet at http://www.cdc.gov/nchs/howto/w2w/w2welcom.htm.

For information on obtaining birth documentation for a birth that occurred outside of the United States or U.S. territories, you can find information on the sites listed below:
  1. If you are requesting a copy of the birth certificate on file in the country of birth, you may contact the U.S. Embassy in that country or that country's consulate located in the United States.

From the North: From I-75 and I-85 go south toward Atlanta. Take exit 248A for Martin Luther King Jr. Drive toward state Capitol. Stay on the right lane and turn right at Jesse Hill Jr. Drive. Go about 0.2 mile. Aldredge Health Center is at the corner of Jesse Hill Jr. Drive and Gilmer Street, across from Grady Memorial Hospital.
