FC Home Adult Day Program

Adult Day Program

The Adult Day Program serves the needs of seniors with functional challenges in a supervised setting and provides out of home respite for caregivers.  Social, physical and cognitive stimulation activities are provided as well as two nutritious meals and two snacks, transportation and health services monitoring (blood pressure checks and medication management).  A social Adult Day Program  is located at each of the four Multipurpose Facilities and a Medical Adult Day Program (Vivian T. Minor) is available through a contracted service provider.

General Information

The Adult Day Program:

  1. Is licensed through the Georgia Department of Community Health (DCH) and complies with the DHR standards and Fulton County Policies and Procedures, as well as the program specific requirements.
  2. Service Hours: Monday through Friday from 7:00 a.m. to 6:00 pm. There are no weekend hours.
  3. All services occur in an environment that promotes safety, respect, dignity, consideration, choice and diversity.
  4. Activity programming  takes into consideration individual differences in health and functioning, lifestyle, ethnicity, religious affiliation, values, experiences, needs, interest, abilities and skills.
  5. Nutrition services, including education and counseling are provided for participants and caregivers, when appropriate.
  6. Social setting: provides up to minimal assistance and supervision (including the use of visual cues with persons with dementia) as needed with activities of daily living, such as walking, toileting and eating.
  7. Exercise or physical activities provided to each participant are based on their functional status and physician prescription.
  8. Caregiver education and support services are available for all participants’ family members.
