This form is to be used when filling an inventory on an estate.

Inventory for Conservator of Minor
A conservator is a person appointed by the Court to manage the property, subject to the direction of the Court, of an individual who is unable to properly manage his or her affairs. Persons whose property is subject to conservatorship are generally those persons for whom guardians may be appointed. This form contains a listing of inventory and accounting of the ward's assets. It must be prepared and completed by the conservator and must also be submitted to the Probate_Court on an annual basis.
