Report Request
Complete the online form below to request a copy of the medical examiner’s exam/autopsy report. Please allow up to 90 business days from the decedent’s date of death for the Medical Examiner’s report to be finalized. Once the report is finalized, a copy will be sent to you via the method requested email, pickup, or mail. (completed reports will be sent from noreply@vertiq.com).
